What Your Toenail Appearance May Be Telling You About Your Health
What does your toenail appearance indicate about your overall health? From fungal infections to systemic issues, learn when to seek expert care.

Have you ever looked closely at your feet and wondered about your toenails? Your toenails do much more than protect your toes; they can provide a window into your overall health.

At Austin Foot & Ankle Institute, we believe that paying attention to your toenails is key to maintaining your mobility and well-being.

Led by board-certified orthopedic surgeon Pedro Cosculluela, MD, our practice provides expert care for patients in Austin and Bee Cave, Texas. Whether you have a sports injury or notice unusual changes in your feet, understanding the signs can help you take charge of your health.

How toenail appearance reflects underlying health conditions

Toenail appearance can change because of systemic health issues, particularly metabolic conditions such as diabetes.

For example, a thick, yellowed, or crumbling nail often points to a fungal infection, medically known as onychomycosis. What may seem like a minor cosmetic issue can be highly dangerous for individuals with diabetes.

A 2024 study found a 31.4% global prevalence of fungal nail infections among adults with Type 2 diabetes. High blood sugar levels can impair the body's immune response and damage peripheral circulation, creating an environment where fungus thrives.

For diabetic patients, an untreated fungal infection can easily lead to severe complications, including deep tissue ulcers and serious bacterial infections.

What do spoon-shaped toenails mean?

Toenails that curve inward to create a cup-like appearance — a condition called koilonychia — often indicate severe iron-deficiency anemia. When your body lacks sufficient iron, it can’t produce enough hemoglobin to carry oxygen to your tissues. This lack of oxygen weakens the nail matrix, causing the nail to collapse.

Anemia affects about 13% of all females in the US. If you notice your nails becoming thin, brittle, and spoon-shaped, seek a medical evaluation rather than treating the nail surface.

Are vertical ridges on toenails a sign of danger?

No, mild vertical ridges running from your cuticle to the tip of your nail are a typical sign of aging, not a medical emergency. Many people worry that these lines indicate a sudden illness or vitamin deficiency, but they’re common as we grow older.

Vertical ridges are essentially the nail equivalent of skin wrinkles. They occur because the nail matrix produces keratin less evenly over time. Unless these ridges are accompanied by severe pain, splitting, or crumbling, they don’t require medical treatment.
